Memory #11: Madoka Death #2
(Ep. 10, significant negative; won in Game #66. Item: a small gear that can be twirled around the finger [reusable, 6/7 uses remaining])
*It's a scene Homura has seen before: the city of Mitakigahara, wrecked and flooded in the wake of the Walpurgis Night. And, just like last time, Kaname Madoka lays amongst the rubble. She's not dead, but she's wounded, exhausted, and clearly in great pain. Her Soul Gem lays in one of her hands, its former shine blotted out by magical pollution. Kyubey is watching, impassive as always.*
Homura: What's wrong? Hey, Kaname-san! Hang on!
*Madoka's agonized writhing only grows more intense, however, and her skin is pale and clammy.*
Madoka: W-Why?!
*With that last word, Madoka lifts the hand holding her Soul Gem towards the sky--but it's a Soul Gem no longer. It has transformed into a Grief Seed, which collapses in on itself and spews forth a roiling black cloud...which soon begins to reshape itself into a monstrous figure. Homura watches on in horror and bewilderment.*
Homura: What?
*The ground shakes as Madoka's witch-form resolves itself, a huge mountain of a monster with two hands stretched towards the yellow, cloudy sky.*
Homura: Why? Whis is this . . . ?
*And, realizing she's failed, Homura steps back in time once again . . .*

This is the piece of the "what went wrong" puzzle Homura had been bracing herself for. She already figured out she must have rewound time more than once, and she's been . . . well partially desperate to know and partially dreading it. But here it is, the big secret--magical girls turn into witches. This memory doesn't give her the exact mechanism for that (since she had no idea what was happening), but it reveals the basic fact of the transformation.
